☑️ Choose your student's activity for 2026/2027
☑️ Update student forms
☑️ Mandatory forms that need signed for Sports Physicals are Sports Physical Treatment Authorization Pediatrics and Physician Questions for the OHSAA PPE.
☑️ These forms will be printed for your student if needed for the Sports Physical appointment.
☑️ Students who do not have these forms completed will not be eligible for this opportunity for Sports Physicals.
☑️ Forms can be printed at the OHMS/HS if you contact the office.
